Below, our new patient forms are available for you to fill out and bring to your appointment.
If you are unable to complete the paperwork for any reason, be sure to arrive at least 15 minutes before your appointment to complete your needed paperwork.
What to Expect
During your initial appointment, you will meet with our expert staff who will discuss your allergy history and conduct a physical exam. This exam is a component of the allergy skin test. The appointment will take 1-2 hours.
For convenience, we also offer same-day initial testing and consultation. At your appointment you will also have same day results and you will be able to speak to your Texas allergist about the available treatment options, which we hope will relax you and assure you in our process.
Be sure to review the list of prohibited medications below, before receiving the allergy test and discontinue for 5 days prior of your test date any beta blockers, tricyclic antidepressants, antihistamines or other prohibited medicated.
Cancellations, Rescheduling and Arriving Late
We understand there are times when you must miss an appointment due to unexpected circumstances. However, when you don't call to cancel an appointment, you may be preventing another patient from getting much-needed treatment.
There is a $50 fee for no-show appointments or appointments canceled with less than 24 hours notice. For procedure appointments there is a $150 cancellation fee. This will not be covered by your insurance company and must be paid prior to rescheduling.
If you are delayed and arrive more than 15 minutes past your scheduled appointment time, we may have to reschedule your visit for the next available time to ensure the safety and prompt care of every patient.
